• Family Registration and Emergency Information, signed and dated
• Child’s Preadmission Health History – Parents’ Report (LIC 702), signed and dated
• Child’s Preadmission Health Evaluation – Physician’s Report (LIC 701), signed and dated
• Consent for Medical Treatment, signed and dated
• Immunization Records for non-school- age children
• Contract & Rate Agreement, signed and dated
• Notification of Parents’ Rights - (LIC 995) receipt, signed and dated
• Personal Rights – (LIC 613A) receipt, signed and dated
